Transcript Request
If the last school you attended is an inactive/closed building, call Student Assignments at 216-523-6347 to request your transcript copy or complete this form and fax to Student Assignments at 216-523-7950 or mail to:
Office of Student Assignments/Records Center
1440 Lakeside Ave., #106
Cleveland, OH 44114
Schools still in operation maintain their own records. If you need a copy of your school record or transcript from a school that is active/open you should contact your last school of attendance directly.
During the summer, when all school buildings are closed, please contact the Office of Student Assignments for transcripts. If you attended a school that is open/active during the school year, Student Assignments can only provide a Letter of Graduation if your birth year is before 1971. For birth years after 1970, Student Assignments may be able to retrieve your transcript from our electronic database*. If you need a copy of your student record, you must wait until the school building reopens in the fall. We apologize for any inconvenience this may cause.
Requests take up to 15 business days to process upon time of receipt, and verification of your birth date is required via a copy of your driver's license or birth certificate.
*Please note that student records were not stored electronically prior to 1990.
